Our E-Z Referral Program makes referring patients simple!

We understand the demands placed on clinicians and social workers supporting people with medical conditions at home.

You can rest assured your patients will receive continuity of your good care, as outlined in our 5 Point CompleteCare Promise.

We promise your patients will receive quality, brand-name medical supplies and fast, reliable delivery right to their door; covered supplies are shipped at no charge. We accept assignment of benefits, file insurance claims, and secure prescriptions and other medical forms. Most importantly, our Customer Care Representatives call to make sure your patients received their order, that the order is correct and they are satisfied and knowledgeable in the use of their supplies. If they have any questions or concerns regarding their supplies, they may call our toll-free number to speak with a Registered Nurse.

To refer a patient, simply complete the online referral form. A Customer Care Representative will contact your patient to ensure we have all appropriate documents. Once we receive the referral form, complete with the signed Assignment of Benefits portion, your patient can expect to have their medical supplies delivered within 1 to 2 business days.
